Your feedback helps make care better

Use this form to tell us about an experience of care at the surgery- it can be good or bad or a concern about the service. CQC uses this feedback to understand the quality of care they get from services at the Surgery. It helps make care better for everybody.

Mental Health Month 

Loneliness and our mental health

Feeling lonely is something that all of us can experience at any point and can have a huge impact on our wellbeing.

Sometimes admitting we feel lonely can be hard but it’s important to remember that many others experience feelings of loneliness too, and that this feeling can pass.

This Mental Health Awareness month, we are encouraging people to reach out to others to help someone who may be feeling lonely, which might in turn help you feel less lonely too.

Friends & Family Test

If you have recently visited the surgery and would like to give feedback on your visit, please click the above link or on the Friends & Family Test link on "have your say" tab on the right. You can also complete a paper version in the practice if you prefer. The NHS Choices website also has a review page if you want to put something on there.  We are more than happy to take action where we can to improve our service and the experience for our patients and we are absolutely delighted when we receive praise!

We can also offer patients appointments in the evening, weekends and Bank Holidays at our Local Hub based at Spring Hall Medical Practice (Near Asda) and Rosegarth also offer early Morning appointments.  Please ask for one of these appointments if they are more convenient for you.  

Diabetic Eye Screening Programme

Have you attended your Diabetic eye screening appointment? 

The NHS provides FREE eye screening for people with diabetes aged over 12 years old. 

Eye screening is an important part of your diabetes care. Retinopathy often has no symptoms until the disease is advanced. If left undetected, it can lead to blindness. To book an appointment, call 01274396900.
